This symposium aims to advance clinical care by discussing updated guidelines for managing refractory/resistant CMV and intolerant patient populations in post-transplant settings. The symposium will highlight the clinical relevance of early CMV identification and robust viral load monitoring, enabling recognition of refractory signals, assessment of viral kinetics, detection of recurrence patterns, and supporting timely clinical decision-making in patients with resistant, refractory and treatment-intolerant CMV. Insights from emerging real-world data and clinical trial evidence will also be explored to evaluate the role of newer anti-CMV agents, including maribavir, in contemporary practice. Through case studies, the audience will explore the potential impact of maribavir on intervention strategies in complex patients.

Dr Kotton is an infectious diseases specialist dedicated to immunocompromised populations, directing the Transplant and Immunocompromised Host Infectious Diseases Program at Massachusetts General Hospital since 2005. Recognized as an international authority on CMV, she has significantly advanced prevention, diagnosis, and treatment strategies for opportunistic infections. Dr Kotton’s contributions include clinical trial leadership, scholarly publications, and involvement in international guideline development that elevated care standards worldwide. Dr Kotton has served as a voting member of the CDC's Advisory Committee on Immunization Practices (2020-2024), the first dedicated immunocompromised patient representative on this policy-setting panel, and has held advisory roles with UNOS and HHS organizations.

Prof Huang is a professor in the Department of Organ Transplantation, The First Affiliated Hospital of Sun Yat-sen University and is a member of American Society of Transplantation (AST). His clinical and research interests are the pathology of transplanted kidney and the diagnosis and treatment of infectious diseases post-transplant. He holds multiple academic appointments, including membership in the Kidney Transplantation Branch of the China Association for International Exchange and Promotion of Healthcare, the Transplant Medicine Committee of the Chinese Association of Research Hospitals, and several subgroups of the Organ Transplantation Branch of the Chinese Medical Association. He is a corresponding editorial board member of Chinese Journal of Organ Transplantation. He has published more than 40 papers as first or corresponding author in Chinese and international journals, and has co-authored five monographs.

Prof Westall is a Consultant Chest Physician at the Alfred Hospital in Melbourne and Adjunct Professor at Monash University with specialized expertise in CMV infections following lung transplantation. His research interests include innate immune system activation early post-transplant and its influence on clinical outcomes. Prof Westall has represented the lung transplant community as a member of the Transplantation Society International CMV Consensus Group, contributing to the development of the "International Consensus Guidelines on the Management of Cytomegalovirus in Solid Organ Transplantation”. Prof Westall has over 170 published manuscripts and has held significant leadership roles across multiple academic and medical committees, including past-Chair of the Lung Advisory Committee of the Transplantation Society of Australia and New Zealand (TSANZ) and the Steering Committee of the Pulmonary Transplant Council of the International Society of Heart and Lung Transplant (ISHLT), and serves as an inaugural member of the Organ and Tissue Authority's Transplant Liaison Reference Group.
